Ville: emergency access for a difficult airway

A boy with a known difficult airway arrives at an emergency department that has never met him.

Vilhelm “Ville” Söderberg, 13 · synthetic patient.

The situation

Ville, 13, lives in Stockholm and is treated at Karolinska. He has a rare syndrome with a small chin and limited mouth opening, so intubation and airway management are hard, and earlier anaesthetics went badly. He has juvenile spondyloarthritis treated with adalimumab (Humira) and methotrexate, which makes him prone to infection. On a family visit to Gothenburg he develops fever, cough and worsening breathing, and arrives at Sahlgrenska's emergency department, where nobody knows him.

What the receiving team needs before anyone reaches for a laryngoscope
  • The difficult-airway alert and the airway plan
  • The last anaesthetic and what failed
  • Current medication and when each dose was last taken
  • Who to call at Karolinska, day and night
  • That he wants to be called Ville, not Vilhelm

What this page implements
text

The emergency overview puts alerts, the airway plan, immunosuppression and last doses at the top, then who to call. It also checks for what the summary should carry but does not: in the reference data, Ville's preferred name exists only in free text, so a system that reads coded data would call him Vilhelm.

Questions from the organisers
  • How can the airway information be available at Sahlgrenska in an emergency?
  • How can Karolinska and Sahlgrenska share it securely?
  • How should it be presented so clinicians understand it quickly?
  • How do we know medication and complications are up to date?
  • How should the preferred name and personal preferences be documented?
  • How would the same information reach care outside Sweden?
