Arne: hip fracture in Copenhagen, home to Sweden
An 83-year-old with ten conditions breaks his hip visiting his son, and must come home to a plan that is ready.
Open the emergency overview Discharge handover
Arne Bergström, 83 · synthetic patient.
Arne, 83, lives at home in Sweden with home care twice a day. He has heart failure, atrial fibrillation on apixaban, type 2 diabetes on insulin, emphysema, a previous stroke with left-sided weakness, leg ulcers and advanced prostate cancer, takes ten medicines, and had anaphylaxis to penicillin. Visiting his son in Denmark, he falls on the stairs and breaks his hip. At Rigshospitalet the team needs his Swedish data before surgery: the anticoagulant, the allergy and his heart.
- Into Denmark: the last apixaban dose, his weight and his functional status are unknown.
- His son is caught in between, as interpreter and coordinator without a formal right to information.
- Back to Sweden: eight plans in two countries and nothing that ties them together. A discharge summary that has been sent is not a confirmed handover.

The overview puts the anticoagulant, the high-criticality penicillin allergy and heart failure at the top, and names what is missing. The handover view creates the discharge care and support plan in the exchange layer as one FHIR Task per Swedish recipient. The plan counts as complete only when every recipient has confirmed receipt.
